Sports Physicals are Required

All students participating in school sports must have a physical.  The physical will cover the student for twelve months.  Physicals can be scheduled through the health office at school or obtained by the student's family physician.  If the physical is done outside of school, the examining doctor must complete a sports' exam form and submit it to the school nurse prior to the start of the chosen sport season.  "Health History Update" forms must also be completed prior to each sport season and returned to the student's coach on or before the first day of practice.  Students not meeting both requirements will be ineligible to play until the required forms are completed and on file in the health office.
