![]() MPS HEALTH OFFICE Eleanor Petrie RNTRANSFER STUDENTS New [transfer] students in grades one through five require a physical exam within two weeks of entering a new school. Students from out of state have one month to have their physical completed. This can be obtained through your private physician or at the school. The Nurse Practitioner is scheduled one morning per week at MPS. A permission slip will be sent home with the student. Please send it back as soon as possible. CLOTHES NEEDED! If there are any sweatpants, sweatshirts, t-shirts, or shorts [sizes 4 through 10] that are taking up space in your closet-could you send them my way? I like to have a few extra pieces of clothing in case the students need to change. Unfortunately, the few I had last fall were loaned to students and never returned. PLEASE call me first with what you can donate, so that I don't get inundated with a truck full. I only accept the above clothing as I find that these will fit everybody. Please keep the health office in mind when you are clearing out outgrown clothes. Donations of new socks and underwear are accepted. I will not accept used underclothes. Thank you!! CHILD HEALTH PLUS-If your family is in the need of health insurance, please call the Mohawk Valley Perinatal Network, Inc. at 732-4657. Both Child Health Plus and Family Health Plus are health insurance programs for those who need insurance but can't afford it. A representative at the Network can help families sign up for the program. A representative will be in Oriskany Falls on March 10 to assist families in the enrollment process.
